Appointments & Consultations

New patients are always welcome !

Our staff is be pleased to assist you in any way possible and every effort will be made to accommodate your schedule.

Dermatology Appointments

For medical dermatology appointments, please check with your insurance company before your appointment to inquire as to whether you will need a referral from your primary care physician to be seen in our office. We also ask that you confirm your co-payment amount as well as any deductibles you may be responsible for based on your specific insurance plan. Please see our financial policy under the “Patient Forms” section below for more detailed information regarding payment. We ask that you bring your insurance card, a photo ID, and your pharmacy information with you to your appointment.

Laser/Aesthetics Consultations and Appointments

Please contact our laser/aesthetic department to consult with our laser nurses and estheticians regarding services we offer. Consultation fees are applicable for non-patients only. Fee will be waived if a treatment is performed.

Plastic Surgery Appointments

When contacting our office regarding our plastic surgery services, we ask that you set up a consultation appointment with Dr. Chung. She will take the time to address your questions and concerns prior to scheduling the actual service. Surgical consultations are subject to a fee, which can be waived if you are treated that day.

Emergencies

Our office provides an on-call service for emergencies only, 24 hours-a-day, 7 days-a-week by calling (617) 731-2390.

Referrals

If you need a referral for your appointment with us based on your specific insurance policy, please contact your primary care provider prior to your appointment. Please make sure to give them the specific date and time of your appointment. You may also considering contacting our office the day before your actual appointment to confirm that we have received the referral.

Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
